Environmental and Health Services Bureau of Vital Statistics Dallas City Hall Regular Business Hours (starting August 1, 2007) Metered parking is available on the south side of Dallas City Hall. Birth and Death Certificates The Bureau of Vital Statistics provides copies of certified birth certificates to persons born in the city limits of Dallas. Death certificates for persons who died in Dallas during the period April 1983 to present are also available. For deaths that occurred prior to April 1983, contact the Bureau of Vital Statistics, Texas Department of Health, P.O. Box 12040, Austin, Texas, 78711-2040. We can only issue the short-form birth certificates for Dallas births that occurred between 1926 and March 1983. If you are requesting a long-form birth certificate for years 1926 to March 1983, this record must be requested from the Bureau of Vital Statistics in Austin. You will need to specify you are requesting the long-form birth certificate or you will receive the short-form birth certificate. Birth and Death Certificates - Identification Requirements The identification requirement for a qualified person to obtain a birth or death certificate is ONE of the following: (1) valid driver's license, (2) state/county/city employee identification card, or (3) Matricula Consular card. If you do not meet these criteria, please contact our office. Only a properly qualified person can obtain certified copies of birth and death certificates. Birth and Death Certificates – Fees Birth certificates, $23 per copy Payment Options When requesting in person cash, money order or cashiers check are acceptable. Birth certificates can usually be obtained from the Bureau of Vital Statistics for persons born anywhere in Texas from 1926-1937 to the present. Birth and Death Certificates - Third Party Requests To obtain a copy of someone else's birth or death certificate, you must be a properly qualified applicant such as an immediate family member (e.g., father, mother, brother, sister, spouse, grandparent, stepparent or stepsibling) or a legal or personal representative with a notarized letter of authorization and a copy of the applicant's identification.
